High Risk Care Management

The goal of the UBA Care Management program is to improve behavioral healthcare outcomes and quality of life for patients requiring coordination of care. Once patients requiring case management are identified, we reach out and collaborate with the providers to develop a care plan. We assess resource utilization, past and present treatments and services, and proactively identify situations that may become barriers to achievement of goals. UBA has developed a number of key initiatives to support the hospital discharge planning process, and we hold re-admission rounds to prevent avoidable re-admissions of recently discharged members.

Care management activities are designed to respond to a member’s needs on a proactive basis when conditions or diagnoses that require alternative, complex, and/or extended treatment arise. Through the care management process, behavioral health care resources are coordinated to ensure that the member receives quality health care in the most cost-effective setting. Members requiring care management are identified based on high risk indicators, diagnostic triggers, utilization patterns, and through direct referral by a behavioral health care provider, primary care or specialty physician, behavioral health care staff, EAPs or other community agencies. UBA has extensive experience in the development of clinical guidelines and continually refines these guidelines.
